Author Biography - Milton J. Lewis

Milton J. Lewis was a Senior Research Fellow in the School of Public Health, 1989- 2006 and is now at the Menzies Centre for Health Policy, University of Sydney, Australia. Kerrie L. MacPherson is an Associate Professor of History, the Kadoorie Institute, University of Hong Kong.
